What Is THC?

  • THC (tetrahydrocannabinol) is the primary psychoactive compound in cannabis
  • It binds directly to CB1 receptors in the brain, producing pain relief, euphoria, altered perception and sedation
  • In medical formulations, THC is most useful for neuropathic pain, spasticity and nausea
  • Products range from low-THC (5–10%) dried flower to high-THC oils (25%+)

THC is the compound responsible for both the therapeutic effects and the psychoactive side effects of cannabis. Prescribers carefully balance the two, seeking the lowest effective THC dose to minimise intoxication while maintaining clinical benefit.

What Is CBD?

  • CBD (cannabidiol) is non-intoxicating — it does not produce a “high”
  • It does not bind directly to CB1 or CB2 receptors but modulates the endocannabinoid system indirectly
  • CBD has demonstrated anti-epileptic, anti-anxiety, anti-inflammatory and analgesic properties in clinical studies
  • Epidyolex — a pharmaceutical-grade CBD oil — is the only NICE-approved cannabis medicine for childhood epilepsy in the UK

CBD is increasingly understood as a multi-target compound. Its value lies not just in direct receptor activity but in modulating the effects of THC, reducing anxiety and paranoia that higher THC doses can sometimes produce.

How Do THC and CBD Work Together?

  • The “entourage effect” theory suggests that cannabinoids and terpenes work synergistically rather than in isolation
  • A balanced THC:CBD ratio (e.g. 1:1) may provide stronger pain relief with fewer psychoactive effects than THC alone
  • High-CBD, low-THC products are often used for anxiety and daytime use where cognitive clarity matters
  • High-THC products are more suited to evening use, severe pain or sleep disorders

Product selection in UK clinics is increasingly guided by condition profile and patient preferences. Your prescribing clinician will discuss the therapeutic rationale behind any ratio recommended for you.

CBD Supplements vs Prescribed CBD

  • Over-the-counter CBD supplements (oils, gummies) sold in UK shops are legal but not medicines
  • They contain CBD under 1mg per serving and cannot make medicinal claims
  • Pharmaceutical CBD (Epidyolex, prescribed CBMPs) is manufactured to GMP standards with verified potency
  • A patient cannot substitute a prescribed CBD medicine with a shop-bought supplement

The quality gap between pharmaceutical CBD and retail CBD products is significant. Pharmaceutical preparations have verified bioavailability, known composition and clinical trial data behind them. Retail products offer none of these assurances.
